Forms have trouble recognizing fields sometimes. what is best way for AI to understand inputs?

Is it best in the prompt to describe each form field then put brackets like this {{describe input}} so the AI understands better?

Out of curiosity..without using that structure, are you seeing any inconsistencies or errors in how the AI reads or uses the form fields?

@abhi Yes, it has inconsistencies and errors without that structure, even when putting specific instructions in both the prompt and the model reminder section. What is the proper structure to make sure form fields send the proper data to the AI to analyze to formulate an output? Is it {{word_word}} or {{word word}} in the prompt next to input blue fields, or must it be in the input blue fields? The way that works best is using radio buttons with choices, but sometimes you need a field with a custom answer from the user. I find that sometimes the AI has to ask again what input is for which field. It can’t figure out the order of input even though I told it what order. It asks clarifying questions after a form is submitted, which it shouldn’t if it really understood, and this is in ChatGPT 5.2.

to be honest, I haven’t personally seen a drastic or consistent difference just based on structure alone. In my own tests, things have generally worked fine either way.

That said, if you have a Loom video, screenshots, or any write-up where you’re seeing a clear difference because of a specific structure, I’d genuinely love to look at it. It would help a lot to understand what’s happening in your setup.

Also curious to hear from others here. If anyone has observed a clear pattern where a certain form structure consistently improves how inputs are understood, please jump in and share.
